Real Madrid took the lead through Karim Benzema's 21st-minute penalty, but Chelsea equalised through Timo Werner's strike just two minutes later.
The match was played at the Alfredo Di Stefano Stadium in Madrid, with Chelsea playing without injured striker Romelu Lukaku and suspended midfielder Mason Mount.
Real Madrid had the better of the chances in the first half, with Benzema hitting the post and Vinicius Junior going close on several occasions.
Chelsea improved after the break and were rewarded for their pressure when Werner equalised in the 27th minute.
The second leg will be played at Stamford Bridge on April 12.
